Baby Dedication

Thank you for registering for Baby Dedication at Pinelake Madison on September 7th at 6:00pm! Please complete this form in its entirety. Upon review of your registration, please list your name and email address. By completing this registration, you are also registering for the mandatory parent class on August 28th during the 11:00 am Service at Pinelake Madison. You are welcome to utilize Little Lakers and Pinelake Kids for two hours in order to worship and attend the class. We are thrilled to celebrate with your family as you commit to raising your child(ren) to know Jesus.
